Usually a face lift will take several hours. Each surgeon is different in the way he performs the procedure. Whereas some surgeons prefer to complete one side of the face before moving to the opposing side, other doctors choose to move back and forth throughout the procedure. Incisions are generally begun above the hair line at the temples and continue along the natural line in front of the ear, then proceed behind the ear lobe and finally to the lower scalp. If work is needed on the neck, a small incision under the chin is made.

After making the incisions, the surgeon will separate the skin from the underlying fat and muscle. Fat cells can either be trimmed or suctioned from around the neck and chin, which improves the contour. The muscle and membrane is tightened, and the skin is pulled back. The excess skin is then removed. The surgeon will use stitches to close the incisions and to secure the layers of tissue, and may use metal clips on the scalp. Following surgery, a drain tube is placed behind the ear, under the skin, and the head may be wrapped loosely in bandages. The tube is to drain any blood which might collect after surgery, and the bandages help to minimize swelling and bruising.
